ff 1.5
MS intelli mouse explorer 3.0

Problem: I "middle click" a link just once, I get two/three... sometimes up to four new tabs open instead of the single tab you would usually expect.

It is really really bothering me (this only started happening a few day ago, nothing has changed, no new extensions...) should I try and re-install firefox? or how would i go about testing the mouse button out?

Maybe you think you are only clicking once but your fingers are so fast that they are actually clicking multiple times

I am used to closing a tab by middle clicking also. With it acting up like this, it is closing multiple tabs at once (really fucking annoying to lose a page you spent ages looking for)

This aspect kinda has me thinking it is my mouse rather than firefox...

I'm guessing you have tab mix plus installed. uninstall it and reinstall it.
